All Referral Rewards allow to set rewards on first card use OR when card issued to new customer, except the Discount Card.

No. It is not possible to add Referral Rewards to the Starter Plan. You must choose the Business Plan

Technically, yes. But you will run into issues with redemption and managers sharing the same account.

Some settings can be changed after activating the cards. However, promo settings cannot be changed to avoid abuse.
